This add-on can be included with any massage service for an additional $10 -- ideal for addressing chronic pain, tight muscles, or problem areas needing extra attention.
Cancellation Policy
Waiver, Release & Cancellation Policy – Unique Fitness, Massage, and Wellness By participating in services at Unique Fitness, Massage, and Wellness, I acknowledge that I am voluntarily engaging in physical activity, massage therapy, sound therapy, and/or related wellness services which may involve physical exertion. I understand these activities carry inherent risks, including but not limited to injury, muscle strain, or in rare cases, more serious complications. I affirm that I am in good health or have received medical clearance to participate. I agree to communicate any discomfort or concerns during sessions and take full responsibility for my physical condition. I understand that all recommendations are intended to support wellness and are not a substitute for medical advice or treatment. I hereby release and discharge Angela Castellanos, her staff, and contractors from any liability, claims, demands, or causes of action arising from my participation in any service, class, or event. This waiver applies to all current and future sessions unless revoked in writing. Cancellation & Refund Policy: You must provide 24-hour notice to receive a full refund. Any cancellation made under 24 hours will result in a 50% refund back to your card. No-shows will also receive a 50% refund as a courtesy. By signing or submitting this form, I confirm that I have read, understood, and agree to the terms of this Waiver, Release, and Cancellation Policy.
