A Healthy Environment

Massage therapy does not diagnose illness or disease and that the massage therapist does not provide any medical treatments or pharmaceuticals, nor are spinal manipulations part of massage therapy. Understand that massage therapy is not a substitute for medical examination or medical care, and that it is recommended that you are concurrently working with your primary caregiver for any condition that you may have. During your initial appointment, please state all known physical conditions, allergies, medical conditions and medications and keep the massage therapist updated on any changes.

Zero Tolerance

All services provided at Unique Fitness Massage and Wellness are strictly therapeutic and non-sexual in nature. Any act of sexual or other forms of harassment are not tolerated. Unique Fitness Massage and Wellness reserve the right to refuse service to anyone or end a session immediately if the therapist feels threatened or compromised, or if any of the conditions herein are compromised; with no refund, 100% forfeiture of prepaid service. Inappropriate solicitation will result in you being permanently banned and reported to the Kalamazoo Police Department, and civil legal action.

Cancellations and Refunds

*PAYMENT is required at the time of booking for all services, including personal training, massage therapy, bodywork, and any other amenities offered.
*CANCELLATIONS made AT LEAST 24 HOURS IN ADVANCE will receive a full refund.
*LATE CANCELLATIONS (under 24 hours) and NO-SHOWS will be CHARGED 50% of the booked service with the remaining 50% being returned to the client. This helps us respect our therapist time and open spots for other clients.
By booking, you agree to this policy. We appreciate your understanding and support in keeping our schedule fair and organized.

WAIVER AND RELEASE AGREEMENT

Unique Fitness Massage and Wellness

Location: 1216 E Stockbridge Ave Kalamazoo, Michigan

By booking any service through Unique Fitness, Massage, and Wellness—including but not limited to massage therapy, cupping, assisted stretching, myofascial therapy, sound therapy, personal training, and fitness classes—you acknowledge that you have read, understood, and voluntarily agree to the following terms and conditions:

1. Assumption of Risk

You understand that both massage therapy and personal training involve physical activity and/or bodywork that carry inherent risks. These may include, but are not limited to, muscle soreness, bruising, physical discomfort, aggravation of pre-existing conditions, or injury.
You acknowledge that you are voluntarily participating in these services and assume full responsibility for any physical, mental, or emotional harm that may result. You agree to communicate all relevant medical conditions, injuries, limitations, or concerns to your massage therapist or trainer before any session.

2. Waiver and Release of Liability

In consideration for receiving services at Unique Fitness, Massage, and Wellness, you release, waive, and discharge Angela Castellanos, the facility, its employees, independent contractors, affiliates, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, injury, illness, or damages (including death) sustained by you or your property, whether caused by negligence or otherwise, while participating in any service or activity.

3. Medical Representation

You represent that you are physically and mentally able to participate in massage therapy and/or personal training. You understand these services are not substitutes for medical care or diagnosis, and that neither the massage therapist nor the personal trainer is qualified to provide medical advice or diagnose medical conditions.

You confirm that you have disclosed any relevant health information and agree to consult your medical provider if needed.

4. Rules and Regulations

By receiving services at Unique Fitness, Massage, and Wellness, you agree to abide by the following policies:

Arrive on time for all appointments
Practice proper hygiene
Provide a minimum of 24 hours’ notice for cancellations or rescheduling
Treat staff, clients, and the space with respect
Wear appropriate attire for fitness or therapy services
Follow guidance and instructions from your therapist or trainer

The facility reserves the right to refuse or discontinue service at any time for failure to follow these rules or for any inappropriate or unsafe conduct.

5. Acknowledgment
By booking online, you acknowledge that you have read and understand this Waiver and Release Agreement in full, and that you agree to all terms and conditions. Booking an appointment constitutes a binding agreement between you and Unique Fitness, Massage, and Wellness. You understand that this agreement is in effect for all current and future services booked at this facility.

If you do not agree to these terms, do not book a service.
